Getting Started

Please follow these steps to complete the application process.

1. Print and complete the a W-9 form for tax purposes. Fax or mail the completed form. Our contact information is located here. No affiliate accounts will be approved without a valid W-9 form on file.

2. Create an account in our online shopping cart.

3. Once your are logged in to your new account, click on the "Request Account Upgrade" link located in the upper-right corner "Account Menu" navigation. Use the "Account Type" drop-down to select "Affiliate Account." Click "Submit."

4. Your application will be reviewed and we will respond with a decision within 2 business days. We will send you an email to the address provided notifying you of our acceptance or denial. Please note, we will not process any applications without a valid W-9 form on file first.

If you are accepted, your confirmation email will have detailed instructions on how to log in to your Affiliate account and links to marketing materials.
